Rocket Failure Thoughts from an Astronaut

Recently I went to Kennedy Space Center and I had the opportunity to ask an astronaut (John Creighton) about this thoughts on the recent rocket failures and rocket failures in general.

John Creighton is a veteran astronaut who flew on three space shuttle missions and commanded one. After the Challenger disaster he was put in charge of resuming safe space flight.
